3)a)ii)A) One nonprescription oral medication was not labeled with the child's first and last name. 3)a)ii)A) There was one medication authorization with a brand-name and dosage amount for the liquid form of the medication. The medication that was provided for the child had a different name on the packaging as it was a generic form of the medication and the medication was in tablet form with a different dosage on the label than that written on the medication authorization. 3)a)ii)A) There were two nonprescription medications that had a range of dosage amounts on the label for children ages 6 to 11 years. There was no dosage amount written on the medication authorizations, and there was no medical professional's note with the dosage needed for the two children. 3)c) The kitchen door was left open and thus, the medications stored in the kitchen were accessible to children in care.
